    Introduction The nursing process consists of five phases; Assessment, diagnosis, planning, implementation and evaluation. The process was developed by Ida Jean Orlando in 1958 when she observed that the patient should be the most important part of nursing and that nursing should be oriented toward improving the outcome of the patient's care rather than nursing objectives (Camilleri, 2013). The notion of nursing process emerged in the United States throughout the 1960s, the nursing process was formed by the American context but it was later sent to the United Kingdom in a form limited, and there it was modified to fit a different context. (Cuesta, 1983). The nursing process is used to help assess patient needs and also produce a sequence of actions to report and resolve patient problems. The nursing process is neither a model nor a philosophy because it is something defined but it is basically a logical thought process and it must be practiced by nurses because it is the common part of the practice that unifies different types of nurses who work in different fields. because the essential part of nursing practice is providing holistic and patient-centered care. (Roper, Logan, Tierany, 1996 p.51). Assessment Assessment is the first phase of the nursing process and refers to the individual's ability to identify the ongoing nature of the condition. The assessment includes: collecting data from or about the patient, for example their vital signs, reviewing the collected information, recognizing the patient's problem and also detecting the importance of the problems.
